PSYCHIATRIC  EVALUATION/CONSULTATION    FOR

      CHILDREN, ADOLESCENTS, AND ADULTS

The first appointment with me is the initial evaluation or consultation.  It is to understand the presenting problems or concerns you or your child is having, to obtain the history in regards to this problem, and to gather a medical history.  Sometimes it's not easy to articulate the problems one is having, or to understand the problem thoroughly in one visit.  It may then be determined that additional appointments are necessary to do so.

At the end of our initial visit, we will clarify together if we have a good assessment of the presenting problems to form a diagnosis of the problem, and a psychiatric diagnosis should one exist.  We will try to identify its possible causes and explore treatment recommendations.  In regards to treatment, as I tell all of my patients, I see my role as collaborator.  I provide you with information regarding available treatment options and of those options I offer my recommendation however you decide what option(s) is/are right for you.

MEDICATION MANAGEMENT
FOR
CHILDREN, ADOLESCENTS, AND ADULTS

As I frequently acknowledge with patients, medications can help with symptoms someone is having (ie depressed mood, poor energy, symptoms of panic or anxiety, insomnia, etc) , but not what causes one’s symptoms, which typically only psychotherapy can provide.

Having symptom relief, however, can enhance one’s quality of life, may allow engagement in one’s life that was previously inhibited, and permit improved global functioning. For some, medication alone may show some benefit but only have limited value and research continually points to combination of medication and psychotherapy as providing better outcomes than medication alone.

Medication treatment for psychiatric disorders in children is compelling and controversial. There is much found in the media presently on these topics- some good (heightening awareness of things like suicide, depression, eating disorders, etc in young people), and some less helpful (eg shaming certain treatment choices like medication). At the forefront of this is the issue of safety.  The potential risks of utilizing medication in young populations must be weighed against the risks of untreated illness/problems.

When possible, priority and preference should always be given to psychosocial interventions first, like psychotherapy or behavioral therapy, before giving consideration to medication. However medication has been found to be helpful in pediatric populations in psychiatric disorders like ADHD, Anxiety Disorders like Obsessive-Compulsive Disorders, Autism, Bipolar Disorder, Major Depressive Disorder, and Schizophrenia.

If medication is determined to be likely beneficial for you, or your child, and it is something that you are also interested in as a modality of treatment, we will work together to choose the best medication treatment.

Unfortunately, not every patient responds with medication and for some it can take some time to find the right medication, however I will work collaboratively with you providing the potential risks and benefits of every appropriate medication option available, and follow up with you closely, so you can feel comfortable in making the right choice for you.

                      PSYCHOTHERAPY

I maintain a limited psychotherapy caseload (adults only) so my availability for psychotherapy varies.

Treatment of any kind with me always starts with an initial consultation, which can take 1-3 hours, depending on the nature of the problem and availability. The goal of this appointment is to get an understanding of the underlying issues that contribute to your presenting problems, gather if the problem is one I can help you with and, particularly in the case of psychotherapy, for the both of us to assess if my approach is right for you.

I work in an intensive short-term (from 1 session to 1 year) dynamic model of psychotherapy which requires both our active participation and aims beyond symptom reduction, and towards your full vitality and genuine engagement in your life in your unique way. It is for you, and I, to know you as honestly and deeply as possible- for you to know what you feel, how you feel it, and see what you do with your feelings which either brings greater connection to you and others, or disconnection and suffering.

If we determine you may be best served with an alternative approach, or if short-term treatment is not possible, we will discuss appropriate referrals so you can continue to move towards healing and wellness as quickly as possible.
